A Clinical Review of Upper Gastrointestinal Tract Bleeding

Abstract
A total of one hundred fifty eight cases of upper gastrointestinal tract bleeding were clinically
reviewed, who were admitted to Kyoungpook University Hospital during six years from April 1962
to March 1968.
The Following Results Were Obtained:
1) The incidence of upper gastrointestinal tract bleeding was 3.92% of all patients (4030 cases)
admitted to our surgical department.
2) The predominant diseases producing upper gastrointestinal tract bleeding were peptic ulcer
(53%), stomach cancer (22.8%), esoph-ogeal varices (13.5%) and gastritis (5.1%) in this order.
3) The age and sex incidence of gastrointestinal tract bleeding revealed a greater prevalence in
male, 4th decades, with a ratio of 4 males to 1 females.
4) On the whole, combined hematemesis and melena more frequent in peptic ulcer while hemat
emesis was frequent in esophageal varices and gastric cancer.
5) Among 158 cases, 87 cases (55.1%) had massive bleeding with a mortality rate of 18.4% (15
cases), 71 cases (44.9%) had moderate bleeding with a mortality rate of 15.5% (11 cases) and no
cases had mild bleeding in this review.
6) Among 158 cases, 129 cases (81.6%) were operated and 29 cases (18.4%) were not operated.
The mortality rate for patients receiving surgical treatment was 16.2% (21 cases), while for the
conservatives were 20.7% (6 cases).
7) Among operated cases (129 cases), 92 cases (71.3%) were operated in emergency with 18.4%
(17 cases) of mortality rate, while 37 cases (28.7%) in elective operation with 10.8% (4 cases) of
mortality rate.
Wound comlications, recurrent bleeding, plumonary complications and leakage were the most
common complications after surgery and they occured in 31 cases (24.0%)of the total 129 cases.
8) The over all mortality rate was 17.1%.
9) Among 27 cases of death, the causes were mainly due to surgical complications (8 cases),
sustained shick (5 cases) and sepsis (4 cases) in this order.
